The Round 2 Questionnaire

This interactive website represents a synthesis of your answers in Round One and includes 241 items for your consideration. Each item is accompanied by a dropdown box containing answers provided in Round One. The dropdown box is activated by clicking on the down arrow key. Choose one of the answers from the list by clicking on it.

At the end of each part of the questionnaire, a "SUBMIT" button allows you to send your responses electronically, and a "proceed" option allows you to continue through the three parts of this questionnaire. You must execute the "SUBMIT" button at the end of each part of the questionionnare or your responses will not be sent. You may, however, complete the questionnaire in parts.
